Please enter information at each following section;

If you do not have a history of your arrival in Japan or past applications for a Certificate of Eligibility, you do not need to register.

If you do not have family members living in Japan, you do not need to register "Family members residing in Japan and persons living with you".

Item	Notes
Hometown Address (Current Address)	Enter the official address of where you currently live
Family register Address	If your current address is different from your official address of family register, please enter it. If they are the same or you do not have family register, leave blank
Place of Birth	Please enter the city name, then Province name. If you have a passport, please enter it according to the notation on your passport.  Ex.) Yokohama City, Kanagawa Prefecture
Intended place to apply for visa	Enter the name of the city where you will apply for your visa. The city must be where the Japanese Embassy or Consulate is located.　 Ex.) Beijing
Port of entry	Please select the airport you plan to use when you arrive in Japan. If the information has already been entered by the school, please leave it as it is if there are no problems
Date of entry	Please enter your expected date of entry into Japan. If the information has already been entered by the school, please leave it
Criminal record	If you have never been punished for a crime in the past, select "No", if you have, select "Yes" and then enter the detail of the crime
Have you ever left Japan under deportation / departure order?	If you have never been deported or ordered to leave Japan, select "No". If you have, select "Yes" and enter the number of times and date of most recent departure
History of visit to Japan	If you have visited Japan before, please enter the date of entry/departure, status of residence, and purpose of stay. If not, please do not enter any information
Past history of apply for a Certificate of eligibility	If you have applied for a Certificate of Eligibility before, please enter the information such as time of application, the status of residence for which you applied, and the result. If not, please do not enter
List of Family members living in Japan	If you have family members currently living in Japan, please enter their information (name, date of birth, nationality, relationship, place of residence, whether or not they plan to live with you, resident card number, and name of their place of work/study)
List of Family members living in your Home country	Please enter information about your family in your home country (name, date of birth, nationality, relationship, place of residence, and name of employer/student)
