Detailed Instructions for Each Item:

  • Hometown Address: Enter the official address in your home country
  • Family Register Address(For CoE Application Only): If your current address differs from your family register address, enter the family register address here. Leave blank if they are the same or if you do not have a family register.
  • Place of Birth(For CoE Application Only): Enter the city and province name as it appears on your passport, if available. (Example: Yokohama City, Kanagawa Prefecture)
  • Intended Place to Apply for Visa(For CoE Application Only): Select the Country and City name where you will apply for your visa. This should be the location of the Japanese Embassy or Consulate.
  • Port of Entry(For CoE Application Only): Select the airport you plan to use upon arrival in Japan. If this information is pre-filled by your school and is correct, leave it unchanged.
  • Date of Entry(For CoE Application Only): Enter your expected date of arrival in Japan. Leave it unchanged if already provided by your school.
• Criminal Record: Select "No" or "Not Applicable" if you have never been convicted of a crime. If you have, provide details of the offense.
• Deportation History(For CoE Application Only): Select "No" if you have never been deported or ordered to leave Japan. If you have, select "Yes" and provide details including the number of occurrences and the date of the most recent event.
• History of Visits to Japan(For CoE Application Only): If you have previously visited Japan, enter details such as dates of entry/departure, status of residence, and purpose of visit. If not, leave this section blank.
• Past Applications for a Certificate of Eligibility(For CoE Application Only): If you have previously applied, include details such as the application date, status of residence applied for, and the outcome. Otherwise, leave blank.
• List of Family Members Living in Japan: For family members residing in Japan, enter their details including name, date of birth, nationality, relationship, place of residence, plans to live with you, resident card number, and place of employment/study.
• List of Family Members in Your Home Country(For CoE Application Only): Enter details about your family in your home country, including name, date of birth, nationality, relationship, place of residence, and employer or school.
